In the process of saving her, recently awakened powers within Ryan accidentally cure her of a pestilent touch, only to turn him into a carrier. When people start dying from what is suspected to be a mysterious biological agent, Mears County, Colorado, and its only town, Packer City, are locked down for a suspected Ebola outbreak. During the quarantine, Mary DeMure learns what she has done to Ryan and must convince him to go with her to see an ancient Navajo Hataalii (a singer of ceremonies, medicine man, and so much more). Only the Hataalii can cure Ryan of his Touch of Pestilence or teach him to control his powers over life and death. During the journey to their ultimate destiny, Ryan and Mary learn who is responsible for their suffering and about a conspiracy beyond the realm of their reality. They discover a cast of mortal and immortal characters-good, evil and somewhere in between-who are pulling the strings from earth and a place called the Afterworld.
